As ESCAPE approaches the end of its seven-year journey, it is time to pause, reflect on what we have achieved, and, above all, say a sincere and heartfelt thank you to everyone who made the project possible. When the first plans for ESCAPE were taking shape in 2019, few could have imagined that we would build such a strong, international and multiprofessional consortium. Seven years later, we can look back with pride at a project that brought together researchers, clinicians, care professionals, patients and public contributors across Europe around a shared goal: to improve care for people living with multimorbidity through an integrated, patient-centred biopsychosocial approach. From coordinating a complex international consortium and developing interventions and training, to creating the digital infrastructure needed to support care managers and establishing and delivering a major clinical trial across 11 sites in six countries, every work package made an essential contribution. The ESCAPE trial ultimately randomised 322 participants, with blinded follow-up assessments continuing until May 2026. The final data cleaning was completed on 31 July 2026, marking another major milestone for the consortium. Behind these headline achievements lies an enormous amount of collaborative work, from legal and ethical preparation, patient and public involvement, training and supervision, to statistical and health economic planning, data management, monitoring, publications, communication and implementation. While the main clinical trial results are mixed, they do not diminish what ESCAPE has demonstrated. We have shown that an integrated, patient-centred biopsychosocial approach to multimorbidity care can be implemented across different European healthcare systems and that this approach is valued and appreciated by patients. We have also created a substantial body of knowledge, infrastructure and practical experience that can inform future research and implementation. As funding draws to a close, the work therefore continues in important ways. We still have analyses and publications to complete, results to communicate and lessons to share. We also have a responsibility to ensure that the investment made in ESCAPE contributes to the future of multimorbidity care, including continued advocacy for better integration of biopsychosocial approaches within health systems.

For those continuing in research, ESCAPE leaves us with new questions as well as new answers: how can the concept be refined, strengthened and adapted to deliver even greater benefit in the future? None of this would have been possible without the dedication of the ESCAPE consortium. To every partner, researcher, clinician, care manager, assessor, patient, carer, public contributor, administrator and supporter: thank you for your time, expertise, patience and commitment!
